Fill out a trial application to authorize us to obtain and disclose your client’s medical information in order to obtain preliminary quotes for complicated cases.

Don’t let less than perfect health stop you from getting your client the coverage so needed. Just provide us with some basic medical history by Requesting a Quote. Well discuss your client's insurability with several different underwriters from several different carriers on a preliminary basis as a ‘Trial App’.

Just download and fill out our Trial App, and we’ll do the rest.

Once we have a tentative offer, we’ll run you all the quotes and illustrations using the tentative rate class. Remember that tentative offers are not a promise of coverage and that the final rating and rate class are always subject to the formal application, age/amount requirements, MVR, MIB, Pharmacy Check, and any other medical records or requirements the underwriter requests.
